Issue 1486: choice-keyed QTI modal feedback

In both cards, pick 8 (choiceB) and submit. The good fixture must render the authored “Dividing 17 by 2…” misconception. The broken twin deliberately has no qti-modal-feedback identifier="choiceB", so it visibly falls back instead.
